Care Transitions – Sun Health Makes House Calls to Aid Recovery
Daniel M. of Surprise was just starting to recover from knee surgery when an unexpected fall landed the avid golfer back onto the surgery table to repair his shoulder. Daniel, who was anxious to return to the greener pastures of the golf course, says he is now back in the swing of things thanks to Sun Health and the outreach he received from the Care Transitions Team.
“It always feels good to have people in your corner,” Daniel shares. “I was lucky that I did not need much, but the calls and check-ins meant a lot to me. It was comforting to have Sun Health at my side.”
Sun Health’s Care Transitions team regularly followed up with Daniel following his hospital stay, answering questions about his recovery, discharge instructions and prescribed medications. They also made sure he was eating the right foods for recovery and made sure he knew they were only a phone call away if he had any needs following his surgery.
Daniel, who is very familiar with Sun Health, once serving a two-year term as chairman of the board, says the frequent check-ins upon discharge from the hospital were comforting and wants to be sure others understand Sun Health’s significant role as a provider of health and wellbeing services in the West Valley.
“I was the chairman of the board for two years, so I do understand Sun Health, but this time I was on the other end and receiving services, so a very different role,” Daniel shares. “Sun Health is all about serving the community (which I experienced first-hand) and all about being a gateway for the community to be able to receive quality health care. The foundation (Sun Health Foundation) supports the local Banner hospitals (referring to Sun Health’s fundraising activities that help provide advanced health care services at Banner Del E. Webb and Banner Boswell medical centers). I want to share with the community their focus and dedication to making sure our community has quality health care.”
Daniel notes that he is well on his way to recovery and the only thing slowing down his golf game these days is not any pain, but instead that, “it takes a little longer to warm up the shoulder before my first swing, but other than that, I am good!”
